Job summary

Local Infusion seeks a skilled Nurse Practitioner (CT-licensed) for a remote, patient-facing role focused on neurology and memory care. You will deliver telehealth services, assess patients, create care plans, and prescribe therapies in collaboration with supervising physicians.

Qualifications

• Active, unrestricted NP license in Connecticut.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• Comfortable managing a full patient panel with physician collaboration.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with Neurologist(s) and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR in compliance with regulatory requirements.
