Local Infusion is seeking a Nurse Practitioner specialized in neurology and memory care to join our remote care team. You will provide direct telehealth care to patients with neurological conditions, collaborate with physicians, and develop care plans.

Qualifications

• Unrestricted NP license in Connecticut.
• 2+ years of clinical experience in neurology, memory care, or dementia.
• Certification in geriatrics, adult-gerontology, or family practice preferred.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Assess patients, develop care plans, and manage ongoing treatment with supervising physicians.
• Collaborate with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate clinical documentation in the EMR meeting regulatory requirements.
