Job summary

Local Infusion is seeking a licensed Nurse Practitioner in Connecticut to join our remote care team focused on neurology and memory care. This role is patient-facing, telehealth-based, and requires collaboration with physicians to improve access and shorten time to treatment.

The NP will assess and manage care plans, educate patients and families, and document thoroughly in the EMR in accordance with regulations. Prior geriatrics or dementia care experience is highly valued.

Qualifications

• Must hold an active CT NP license and related geriatrics certification.
• 2+ years of clinical experience in neurology, memory care, or dementia care.
• Remote telehealth role requiring strong clinical judgment.
• Excellent communication with patients and caregivers.

Responsibilities

• Provide direct clinical care to neurology/memory care patients via telehealth.
• Assess, plan, and manage treatment with supervising physicians.
• Collaborate with neurologists and care team to ensure continuity.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate EMR documentation compliant with regulations.
