Job summary

Local Infusion is seeking a Nurse Practitioner licensed in CT to join our remote-first care team. The role delivers direct telehealth neurology and memory care services to patients, collaborating with physicians to improve access and treatment timelines.

You will conduct assessments, develop care plans, prescribe therapies as allowed, educate patients and families, and maintain compliant EMR documentation while delivering compassionate care.

Qualifications

• Active, unrestricted NP license in Connecticut (CT).
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• Experience delivering care via telehealth is a plus.

Responsibilities

• Provide direct clinical care to neurology/memory care patients via telehealth.
• Assess patients, develop care plans, and manage treatment in collaboration with physicians including prescribing therapies.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate EMR documentation in line with regulatory requirements.
