Job summary

Local Infusion is seeking a Nurse Practitioner to provide remote/telehealth care for patients with neurological conditions and memory disorders. You will work with physicians to develop and manage treatment plans, educate patients and families, and document care in the EMR.

The role requires an active Maryland NP license with 2+ years of neurology/memory care experience; strong communication and independent panel management are essential.

Qualifications

• Active, unrestricted NP license in Maryland; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers

Responsibilities

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
