Job summary

Local Infusion is seeking a Nurse Practitioner to provide telehealth neurologic and memory-care care in a remote, patient-facing role guided by a supervising physician. You will assess patients, create care plans, and manage ongoing treatment with prescription authority and detailed EMR documentation.

You will educate patients and families on diagnoses, treatment options, and disease progression, ensuring continuity of care with the broader team.

Qualifications

• Licensed NP in New Hampshire with current credentials.
• Telehealth clinical experience preferred.
• Ability to collaborate with physicians and care team.
• Strong patient and caregiver communication.

Responsibilities

• Provide direct clinical care via telehealth for neurology/memory care.
• Assess patients, develop plans, and manage treatment with supervising physicians.
• Prescribe therapies and document in EMR per regulations.
• Educate patients and families on diagnoses and progression.
• Maintain accurate and timely clinical documentation.
