Job summary

Daymark Health Inc. is recruiting for a National Virtual Oncology Nurse Navigator (All States). This fully remote role supports oncology patients via phone and virtual platforms, coordinating care and providing education throughout the cancer journey.

Compensation ranges from $85,000 to $100,000 annually, depending on location and experience. The ideal candidate holds a BSN, an active multi-state RN license, 3+ years of nursing with 2+ years in oncology, and relevant certifications or

Qualifications

Bachelor of Science in Nursing required with compact unrestricted RN license and licensure in all 50 states.
At least 3 years of nursing experience, including 2 years in oncology nursing.
Oncology certifications (OCN/AOCN/AOCNS/CCM) strongly preferred or to be obtained within 2 years of hire.

Responsibilities

Provide direct patient care via virtual platforms with high-quality care.
Support the oncology care journey from screening to survivorship or end-of-life care.
Offer consultative clinical support by phone, email, or video; escalate urgent cases to a Nurse Practitioner as needed.
Triage symptom responses and escalate as necessary; address patient needs.
Coordinate care, including nutritional support, discharge planning, and referrals.
Educate patients about their diagnosis, Daymark Health’s offerings, and next steps.
Build rapport with patients and care networks through empathy and active listening.
Collaborate with clinicians and social workers on integrated care and social determinants of health.
Adhere to policies and documentation standards and optimize use of systems and tools.
