Job summary

Equity Staffing Group is seeking MTM Pharmacists for remote work nationwide on a 6-month contract with potential extension to FTE. Start dates: 11/30/2026 or 12/07/2026.

The Clinical Engagement Services Center offers clinical services including Adherence, MTM, diabetes management, and drug utilization reviews. Pharmacists will conduct telephonic comprehensive medication reviews, document care, and coordinate with primary care providers.

Qualifications

Experience implementing MTM programs.
Strong knowledge of disease state management.
Proficiency with MTM platforms and data analysis.

Responsibilities

Conduct telephonic medication reviews and MTM consultations.
Document patient care notes clearly and concisely.
Coordinate with primary care providers and caregivers.
Maintain HIPAA compliance and patient confidentiality.
Support team projects and process improvements.
