Job summary

Equity Staffing Group is seeking a Clinical Pharmacist to work remotely from the Sunrise, FL area, supporting MTM and clinical programs. The role requires a Florida license and at least one year of remote work experience.

You'll conduct telephonic reviews, identify gaps in care, educate patients, document care, and maintain HIPAA compliance while collaborating with a clinical team. Start dates include 11/30/2026 or 12/07/2026.

Qualifications

• Must reside in the Sunrise, Florida area.
• Active and unrestricted Florida Pharmacist license.
• 1+ year of experience working remotely/independently in a cross-functional environment.
• Strong knowledge of disease-state management and the 5 Core Elements of MTM Services.
• Ability to navigate MTM platforms and multiple applications simultaneously.
• Strong communication, documentation, organization, and time-management skills.
• Intermediate proficiency with Microsoft Word, Outlook, and Excel.

Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews with members.
• Review medications to identify gaps in care, adherence concerns, drug interactions, high-risk meds, and therapeutic opportunities.
• Assess medication regimens for effectiveness, dosing, and adverse reactions.
• Provide patient education on medications and disease-state management.
• Communicate clinical recommendations to members and healthcare providers.
• Accurately document patient care, interventions, and recommendations.
• Maintain HIPAA compliance and meet productivity/quality expectations.
