Responsabilités

• Conduct telephonic MTM reviews and comprehensive medication reviews with members.
• Identify gaps in care, adherence concerns, interactions, high-risk meds, and therapeutic opportunities.
• Assess regimens for effectiveness, dosing, and adverse effects.
• Provide patient education on medications and disease-state management.
• Communicate clinical recommendations to members and providers.
• Accurately document patient care, interventions, and recommendations; maintain HIPAA compliance.

We are looking for a Clinical Pharmacist who resides in the Sunrise Florida area and holds an active, unrestricted Florida Pharmacist license to join a Clinical Engagement Services Center supporting Medication Therapy Management (MTM), medication adherence, diabetes, polypharmacy, STARS adherence, retrospective drug utilization, and other clinical programs.

Location: Remote – Must reside in the Sunrise, FL area

Schedule: Monday–Friday | Rotating shifts between 9:00 AM–8:00 PM CST

Key Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews with members.
• Review medications and clinical information to identify gaps in care, adherence concerns, drug interactions, high-risk medications, and therapeutic opportunities.
• Assess medication regimens for effectiveness, appropriate dosing, and potential adverse reactions.
• Provide patient education on medications and disease-state management.
• Communicate clinical recommendations to members and healthcare providers.
• Accurately document patient care, interventions, and recommendations.
• Maintain HIPAA compliance and meet established productivity and quality expectations.

Required Qualifications

• Must reside in the Sunrise, Florida area.
• Active and unrestricted Florida Pharmacist license.
• Strong knowledge of disease-state management and the 5 Core Elements of MTM Services.
• 1+ year of experience working remotely/independently in a cross-functional environment.
• Ability to navigate MTM platforms and multiple applications simultaneously.
• Strong communication, documentation, organization, and time-management skills.
• Intermediate proficiency with Microsoft Word, Outlook, and Excel.

Preferred Qualifications

• PharmD.
• 2+ years of pharmacy experience in retail/community pharmacy and/or PBM; new graduate experiential experience may be considered.
• Previous call center experience.
• Diabetes-related certification.
• Bilingual in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.
• Additional active state Pharmacist licenses are a plus.
