Qualifications

• PharmD or Bachelor of Pharmacy required.
• Ambulatory, hospital, community, or MTM experience is a plus.
• Licensed pharmacists welcomed, including new graduates.

Responsibilities

• Perform CMRs and Targeted Medication Reviews with patients.
• Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
