Job summary

Pharmacy Careers is seeking an MTM Pharmacist to enhance medication optimization through comprehensive reviews and patient coaching. Many roles offer fully remote or hybrid schedules to support work-life balance.

In this role you will conduct CMRs and MTM interventions, collaborate with care teams, document clearly, and help reduce hospitalizations while improving adherence. Licensure and PharmD required; new graduates encouraged.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients on safe and effective medication use and adherence strategies.
• Maintain CMS MTM program compliance and quality metrics.
