Licensed pharmacists with PharmD or Bachelor of Pharmacy are invited, including new graduates. Ambulatory, hospital, or community MTM experience is a plus; strong patient counseling and precise documentation are essential.

Qualifications

  • Education: PharmD or Bachelor of Pharmacy.
  • Experience: Ambulatory, hospital, community, or prior MTM experience is a plus. All licensed pharmacists, including new graduates, are encouraged to apply.
  • Skills: Excellent patient counseling, independent work style, and clear documentation.

Responsibilities

  • Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
  • Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
  • Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
  • Educate patients and caregivers on safe, effective medication use.
  • Maintain compliance with CMS MTM program requirements and quality metrics.
