Job summary

Pharmacy Careers is seeking an MTM Pharmacist to optimize medications and improve patient outcomes. This role offers remote options and flexible hybrid schedules, with opportunities to perform comprehensive medication reviews, coach adherence, and collaborate with care teams.

Licensed pharmacists with PharmD or Bachelor of Pharmacy are invited, including new graduates. Ambulatory, hospital, or community MTM experience is a plus; strong patient counseling and precise documentation are essential.

Qualifications

Education: PharmD or Bachelor of Pharmacy.
Experience: Ambulatory, hospital, community, or prior MTM experience is a plus. All licensed pharmacists, including new graduates, are encouraged to apply.
Skills: Excellent patient counseling, independent work style, and clear documentation.

Responsibilities

Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
Educate patients and caregivers on safe, effective medication use.
Maintain compliance with CMS MTM program requirements and quality metrics.
