Job summary

Pharmacy Careers is seeking MTM Pharmacists to join our Medication Therapy Management team. Conduct comprehensive reviews, coach patients on adherence, and optimize outcomes, often from home or with flexible hybrid schedules.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Licensed pharmacists encouraged, including new graduates.
• Ambulatory, hospital, or community MTM experience is a plus.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
