Job summary

Outcomes is seeking a remote Clinical MTM Pharmacist to deliver patient-centered MTM services via telehealth, telephonic and video visits. You will perform Comprehensive Medication Reviews, manage disease-specific reviews, and coordinate with prescribers to optimize therapy.

Candidates must hold a pharmacy license in one or more states, complete an MTM certificate, and have strong documentation, communication and collaborative skills.

Qualifications

• 1 year minimum work experience in pharmacy or MTM.
• Current pharmacist license in applicable states; multistate licensure possible.
• MTM certificate course completion preferred.
• Board certification preferred (Ambulatory Care, Geriatrics, Pharmacotherapy).

Responsibilities

• Deliver MTM services including Comprehensive Medication Reviews and Targeted Interventions.
• Document and communicate clinical recommendations to prescribers and care teams.
• Assist with cost-effective interventions and adherence strategies.
• Maintain patient records and ensure HIPAA compliance in telehealth MTM.
