Job summary

Pharmacy Careers is hiring a Remote Patient Care Pharmacist to provide telephonic or video-based medication counseling from home. You’ll monitor therapy outcomes, adherence, and potential side effects while coordinating with physicians and care teams to optimize treatment plans.

Ideal candidates hold a PharmD or Bachelor of Pharmacy, possess an active U.S. pharmacist license, and have ambulatory care/MTM experience with strong virtual communication skills.

Qualifications

• Pharmacist with PharmD or equivalent, licensed in the U.S.
• Experience in ambulatory care, MTM, or clinical settings preferred.
• Strong communication and rapport-building skills for remote care.

Responsibilities

• Deliver telephonic or video-based medication counseling to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; suggest optimizations.
• Conduct medication reconciliations, especially for high-risk patients.
• Collaborate with physicians, nurses, and case managers to coordinate plans.
• Document interventions per organizational standards.
• Support population-health and disease-management programs.
