Job summary

Outcomes® is seeking a Clinical MTM Pharmacist to deliver patient-centered MTM services to optimize medication therapy and improve outcomes. Services may be provided by telephone or remote video (telehealth) and include Comprehensive Medication Reviews and targeted interventions.

Responsibilities include documenting recommendations, collaborating with prescribers, and ensuring HIPAA-compliant care. Education requirements include PharmD/ACPE-accredited degree and active licensure; MTM

Qualifications

• Knowledge of MTM core elements (Medication Review, PML, MAP, Interventions & Referrals, Documentation)
• Ability to assess medication therapy for unnecessary, ineffective, inappropriate, or high-risk meds and adherence concerns
• Ability to propose interventions to resolve MTM problems and improve therapy
• Ability to assist with managing medication costs via cost-effective interventions
• Ability to identify and address medication adherence barriers
• Ability to document patient care notes, adverse reactions, and recommendations clearly and professionally
• Ability to collaborate with prescribers and care teams to set disease-state goals
• Provide patient-centered education on chronic diseases and medications
• Strong telephonic communication with seniors and healthcare professionals
• Excellent listening and interpersonal skills with warmth and empathy
• Clear and concise written communication for diverse audiences
• Professional demeanor and interview skills to extract concerns
• Conflict resolution skills for patient interactions

• Knowledge of state pharmacy law and HIPAA privacy compliance

• Strong clinical pharmacotherapy knowledge

Responsibilities

• Deliver MTM services including Comprehensive Medication Reviews and Targeted Interventions to optimize medication therapy
• Communicate and document clinical recommendations to prescribers and interdisciplinary care teams
• Identify and develop patient-specific disease state therapy goals and document care notes clearly
• Support medication cost management through cost-effective interventions
• Provide patient-centered education on chronic diseases and medications
• Collaborate with prescribers and care teams to optimize therapy and transitions in care
• Demonstrate strong telephonic and communication skills for patient and provider interactions

Education

Bachelor's Degree in Pharmacy or PharmD from ACPE-accredited program
