Community Care of North Carolina in North Carolina is seeking a Member Care Coordinator to support care teams, close care gaps, and improve health outcomes for North Carolinians. The role blends remote and on-site work, helping members navigate the health system and access local resources.

You will document referrals, coordinate care, and collaborate with providers while adhering to HIPAA and CCNC policies.

Qualifications

• High school diploma or GED required.
• 2–4 years minimum experience in a health care setting.
• 2- or 4-year degree in health-related field preferred.
• Bilingual preferred.

Responsabilités

• Receive and document referrals into the Care Management platform.
• Outreach, engagement, and scheduling of members for Care Managers.
• Verify eligibility and demographic information.
• Complete appropriate screenings as needed.
• Assist with referrals on behalf of the Care Management or program team.
• Assist with tasks delegated by the Care Management or program team.
• Provide information for access and coordination of resources.
• Assist member with care coordination and health care system navigation.
• Provide culturally appropriate health education and information.
• Provide general education and social support.
• Advocate for members.
