Job summary

Duke HomeCare & Hospice Medical Records Coder II is a certified coder responsible for coding activities for the home health and hospice areas at Duke. Code medical records using ICD-10-CM conventions and review records for specificity and appropriate reimbursement.

Abstract information from records following established methods. Position requires CCS, CPC, or RHIA certifications; 1–2 years of coding experience; remote work with NC address and proximity to office within 2 hours.

  • Anatomy and Physiology
  • Medical Terminology
  • Extensive DRG/APC reimbursement knowledge
  • HH PDGM Knowledge R/T Coding
  • Coding software familiarity
  • Effective written and verbal communication skills

Responsibilities

  • Review complex medical records and accurately code primary/secondary diagnoses and procedures using ICD-10-CM conventions
  • Review documentation and accurately code diagnoses
  • Sequence diagnoses per coding guidelines
  • Abstract and compile data for reimbursement
  • Educate physicians on coding practices and conventions
  • Coordinate with nursing and ancillary staff for documentation
  • Maintain knowledge of anatomy, physiology, and medical terminology for ICD-10-CM coding
  • Understand medical record practices and relevant regulations
