Job summary

Innovaccer seeks a Medical Coder to independently review and resolve front-end claims, applying CPT/ICD-10-CM guidelines and HIPAA requirements in a fast-paced setting. You will work with revenue cycle partners to prevent rejections and promote timely reimbursement.

Requires AAPC/AHIMA certification and 3+ years of coding experience; strong attention to detail and excellent communication are essential. Remote work options available in the U.S. with competitive hourly pay.

Qualifications

• Must hold a current AAPC or AHIMA Certification for a minimum of 3 years.
• Strong knowledge of CPT, ICD-10-CM, medical terminology, anatomy & physiology, and Medicare guidelines.
• Familiarity with proper English grammar, usage, and professional documentation standards.
• Ability to research and analyze data to resolve coding issues.
• Read, interpret and apply policies, procedures, laws, and regulations.
• Read and interpret medical documentation and procedures accurately.
• Demonstrated ability to exercise independent judgment in coding and claim resolution.
• Excellent written and verbal communication with physicians and staff.
• Experience in medical billing with HIPAA compliance.
• Proficient in Microsoft Word, Excel, Outlook, Teams.

Responsibilities

• Average 10 front-end holds per hour.
• Maintain a minimum of 90% coding accuracy.
• Assign ICD-10-CM and CPT codes with modifiers for professional services.
• Review medical records to determine appropriate codes.
• Ensure all diagnosis codes meet medical necessity guidelines.
• Use internal resources and payer guidelines for compliant coding.
• Follow HIPAA regulations and privacy standards.
• Maintain knowledge of laws, payer policies, and guidelines.
• Review and resolve front-end claim holds.
• Participate in meetings with Coding Team Lead and escalate trends.
