Job summary

Innovaccer Inc. is seeking a Medical Coder to independently review and resolve front-end scrubs and denied claims, ensuring accurate coding before submission. You will apply CPT/ICD-10-CM codes, audit medical records, and prepare appeal letters to support timely payment.

The role emphasizes precise documentation, adherence to HIPAA, and collaboration with revenue cycle partners to reduce rejections and improve clean claim rates. 3+ years of coding experience and active certification are required.

Qualifications

• Must hold a current AAPC or AHIMA Coding Certification for at least 3 years.
• Minimum of 3+ years of professional coding experience.
• Strong knowledge of CPT, ICD-10-CM, medical terminology, anatomy & physiology; familiarity with Medicare guidelines.

Responsibilities

• Review front-end scrubs/denials and resubmit corrected claims or appeals.
• Maintain a coding accuracy rate of at least 92%.
• Assign ICD-10-CM and CPT codes with modifiers for professional services.
• Review medical records to determine appropriate codes.
• Ensure diagnosis codes meet local and national guidelines and payer policies.
• Adhere to HIPAA and maintain patient privacy; stay updated on coding guidance.
• Prepare reports and communicate documentation needs with physicians and staff.
• Complete CEU requirements and participate in team meetings.
