Qualifications

• 1 year of clinical experience in primary care, hospital, or specialty setting.
• Experience with EMRs and ability to navigate complex systems.
• HIPAA-compliant home office setup with fast internet.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage a panel of 400–450 patients with monthly outreach.
• Make 50–60 daily outbound calls for care plan support and coaching.
• Collaborate with clinical teams to address needs and gaps in care.
• Document interactions in real time and maintain HIPAA compliance.
