Job summary

Cosán Group is hiring a Care Coordinator working 100% remote from the US. You will manage a panel of 400-450 patients, conduct daily health coaching calls, and collaborate with clinical teams to close care gaps while maintaining HIPAA-compliant communications.

Ideal candidates will have MA or CPhT certification, 1+ year clinical experience, and proficiency with EMRs and multiple tech platforms to support patient outcomes.

Qualifications

• Nationally recognized MA certification or CPhT.
• Minimum 1 year of clinical experience in healthcare.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with 50 Mbps download / 5 Mbps upload.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients with monthly outreach.
• Conduct 50-60 daily outbound calls to provide care plan support and coaching.
• Maintain high call quality and professional communication in every interaction.
• Collaborate with clinical teams to address clinical and social needs.
