Job summary

Cosán Group is seeking a compassionate Care Coordinator for a 100% WFH role in the US. You will manage a 400-450 patient panel, perform monthly outreach, and provide health coaching aligned to patient needs. Strong communication and collaboration with clinical teams are essential to close care gaps.

The role emphasizes HIPAA compliance, documentation, and the use of diverse EMR platforms while maintaining a private home office and high-speed internet.

Qualifications

• Must hold a nationally recognized MA certification or CPhT.
• Minimum 1 year of clinical experience in a primary care, hospital, or specialty setting.
• HIPAA-compliant home office with 50 Mbps download / 5 Mbps upload.

Responsibilities

• Manage an assigned panel of 400-450 patients and conduct required monthly outreach to ensure continuity of care.
• Conduct 50-60 daily outbound calls to patients with care plan support and health coaching.
• Close gaps in patient care through Chronic Care Management, Behavioral Health Integration, and RPM services.
