A growing primary care practice is seeking a detail-oriented Medical Assistant to support Chronic and Principal Care Management programs remotely. This role involves conducting outreach calls, documenting clinical interactions, and coordinating care. Candidates should have a Medical Assistant certification, 2–3 years of experience in a clinical setting, and be bilingual in English and Spanish. The position offers flexible scheduling and pays between $17–$25/hour depending on experience.

Qualifications

• Minimum 2–3 years of experience in a clinical or care coordination role.
• Experience with CCM, PCM, or RPM programs strongly preferred.
• Reliable home office setup with stable internet connection.

Responsibilities

• Conduct monthly CCM/PCM outreach calls with enrolled patients.
• Accurately document all encounters and time logs in eClinicalWorks.
• Coordinate care between specialists and providers.
