Qualifications

Responsibilities

• Conduct monthly CCM/PCM outreach calls with enrolled patients.
• Accurately document all encounters and time logs in eClinicalWorks.
• Coordinate care between specialists, PCP, and ancillary providers.
• Assist with care plan creation and updates under provider supervision.
• Identify and escalate urgent patient concerns to the clinical team.
