Responsibilities

• Receive new patient referrals from clients.
• Create charts for all new patients.
• Input medical information in new chart.
• Help Care Coordinators with patient needs as applicable.
• Foster relationships with their team to ensure the CareHarmony culture is positive and supportive.
• May be asked to assist in conducting health assessments telephonically with patients.
