Job summary

Housecall Providers Inc is seeking a Medical Assistant / MA to support discharge follow-up and care coordination. The role involves contacting patients post-discharge, coordinating with clinicians, and linking families to community resources to improve ongoing care.

Responsibilities include assisting with referrals, chart notes, prior authorizations, and maintaining patient records under HIPAA. A Medical Assistant certification and 3+ years in healthcare are required, with a focus on teamwork

Qualifications

• Medical Assistant or equivalent certification.
• Minimum 3 years of experience in a medical setting, including with medical records systems.

Responsibilities

• Complete outgoing calls to patients after discharge to meet regulatory requirements.
• Coordinate care: contact patients, families, and caregivers per clinician instructions.
• Link patients with community resources and social workers to enable continued community-based care.
