Responsibilities

• Patient Communication: Complete outgoing phone calls to patients after they've been discharged from the hospital to meet regulatory requirements and assist providers in obtaining information prior to visits.
• Contact patients, families, and caregivers per clinician specific instruction.
• Outreach may include coordination of care, test results, referral information, appointment rescheduling, and documented instructions from the clinician.
• Link patient, families, and caregivers with other community resources including HCP social worker to enable continued community-based care and to enhance their quality of life.
• Clinical Support: Assist clinicians with referrals to specialists and orders, including but not limited to home health, hospice, palliative care, diagnostic testing, medical supplies, and equipment.
• Assist Housecall Providers staff with patients on their panel, provide support to clinical team working with assigned providers.
• Work collaboratively with the primary care clinical team to ensure excellent communication and continuity of care.
• Assist clinicians with obtaining prior authorizations.
• May authorize prescription refills for non-controlled drugs if approved by clinicians or in accordance with Housecall Providers standing orders.
• Communicate with primary care clinical team regarding patient hospitalizations and clinician visit requests, or equipment needs.
• Send chart notes, specialist referrals, visit summaries and forms for clinician signatures as requested.
