Job summary

Recover-Care Healthcare is seeking an MDS Coordinator to assist the Director of Nursing and the RN Assessment Coordinator with ensuring that documentation meets Federal, State, and Certification guidelines. This role supports the MDS, CAAs, and Interdisciplinary Care Plan.

The position is Remote/Hybrid, with visits to assigned centers every other week and responsibilities to monitor the RAI process and ensure timely, complete submissions.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the OBRA regulations and Minimum Data Set; knowledge of the care planning process.

Responsibilities

• Assist the Director of Nursing and the RN Assessment Coordinator with regulatory documentation.
• Coordinate the RAI process to ensure timely and complete MDS, CAAs, and care plan.
• Visit assigned buildings on an every-other-week basis.
• Report trends from audits to the Quality Assurance Committee.
• Monitor RAI workflow and Medicaid tracking forms.
• Ensure staff compliance with documentation standards.
• Conduct patient assessments and data collection as needed.
