Job summary

Recover-Care Heartland in Topeka, KS seeks an experienced MDS Coordinator to assist the Director of Nursing and RN Assessment Coordinator with Federal, State, and Certification documentation. The role also coordinates the RAI process, ensuring timeliness and completeness of MDS, CAAs, and the care plan.

This is a Remote/Hybrid position. Responsibilities include tracking, audits, and staff follow-up to assure compliance with documentation standards, and participation in interdisciplinary care

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and MDS.

Responsibilities

• Assist the center in adherence to Federal and State regulations and certification.
• Participate in regulatory surveys and correction of deficiencies.
• Visit assigned buildings on an every other week basis.
• Report trends from audits to the Quality Assurance Committee.
• Ensure completion of the RAI process and interdisciplinary care plan.
• Initiate and monitor RAI process tracking and Medicaid tracking forms.
• Follow up with staff to assure documentation standards.
