Job summary

Recover-Care is seeking a Resident Care Coordinator to assist with ensuring MDS and care plans meet regulatory requirements. You will support the Director of Nursing and RN Assessment Coordinator in accurate documentation and timely RAI workflows.

Responsibilities include coordinating RAI activities, ensuring compliance in PCC documentation, and collaborating with staff to maintain high standards of care.

Qualifications

• Graduate of an approved RN or LPN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and the MDS process.

Responsibilities

• Assist the Director of Nursing and the RN Assessment Coordinator with ensuring documentation meets Federal, State, and Certification guidelines.
• Coordinate the RAI process to ensure timely, complete MDS, CAAs, and Interdisciplinary Care Plan.
• Support regulatory surveys and corrective actions as needed.
• Monitor RAI tracking and Medicaid forms through PCC system.
• Ensure timely completion of care plans and patient assessments.
• Collaborate with staff to ensure continuous compliance in documentation.
