Job summary

VIVA USA Inc is seeking an Analyst, Case Management Field to support the LTSS team in a remote role with substantial travel. The role requires meeting members face-to-face and quarterly visits, managing a full caseload, and coordinating care with various health programs.

You will employ care management tools to educate and support members in making informed healthcare decisions and achieving outcomes, while ensuring regulatory compliance.

Qualifications

• Bachelor’s degree in a human-services field is required.
• Non-licensed candidates with a Master’s degree in a human-services field are acceptable.
• Strong ability to travel 50–75% and conduct field-based activities.

Responsibilities

• Evaluate members’ needs and determine a plan for case resolution.
• Coordinate and monitor care plan activities and progress.
• Provide coaching and support to empower members in healthcare decisions.
• Escalate quality of care issues through established channels.
• Utilize care management tools to support outcomes and regulatory compliance.
