Job summary

A-Line Staffing Solutions seeks a LTSS Service Care Manager to oversee a caseload of healthcare members with LTSS needs, combining remote administration with substantial field work.

Role requires regular face-to-face member visits and coordination with caregivers, providers, and physicians. Travel up to 2.5 hours from home is common, with mileage reimbursement; training occurs and is 3.5 weeks long.

Qualifications

• RN and/or LCSW licensure or qualification as applicable.
• Valid driver's license.
• 2+ years of physical health care management experience.
• Willingness to perform extensive field work.
• Willingness to travel for face-to-face member visits.
• Ability to work a Monday–Friday 8:00 AM–5:00 PM EST schedule.
• Ability to work remotely and conduct administrative duties independently.

Responsibilities

• Manage a caseload of healthcare members with LTSS needs.
• Conduct monthly and quarterly member contact.
• Complete member assessments with members, caregivers, or providers.
• Document member assessments and notes.
• Monitor delivery of services through in-person visits and telephonic contact.
• Coordinate referrals for services and ensure no gaps in provider services.
• Collaborate with discharge planners, physicians, and other parties.
