Job summary

VIVA USA Inc. is seeking a Field Analyst for Case Management to support Long Term Services & Supports (LTSS). This role requires traveling 50-75% to meet members face-to-face, managing a full caseload, and conducting quarterly in-person visits to ensure quality care and contract compliance.

Responsibilities include evaluating member needs, coordinating care plans, and collaborating with clinicians. Strong Microsoft Office and EHR skills are expected, with two years of case management experience

Qualifications

• Bachelor’s degree in a human-services field (e.g., sociology, rehabilitation counseling).
• Master’s degree in human-services preferred.
• Minimum of 2 years of case management experience.
• Experience working with HIV/AIDS population preferred.

Responsibilities

• Evaluate referred members' needs and eligibility to determine care resolution and required services.
• Coordinate and implement assigned care plan activities and monitor progress.
• Provide coaching, information and support to empower members to make informed healthcare decisions.
• Ensure compliance with regulatory guidelines and company policies through proper documentation.
