Job summary

A-Line Staffing Solutions is seeking an LTSS Service Care Manager (RN or LCSW) in North Carolina for a Hybrid Field/Remote role. You will manage a caseload, perform assessments, develop care plans, and coordinate services with providers and discharge teams.

The position involves travel up to ~80%, field visits, and remote documentation time. Hybrid schedule includes 1–2 remote days and 3–4 field days per week, with quarterly in-person team meetings in the Raleigh/Charlotte area.

Qualifications

• Active RN license and/or LCSW / LCSW-A current and valid.
• Valid driver’s license and ability to travel for member visits.
• 2+ years of physical health care management/case management experience.
• Strong documentation skills and comfort with electronic health records.
• Solid computer skills including Microsoft Office and virtual work environments.

LTSS Service Care Manager (RN or LCSW) – Hybrid Field/Remote (NC)

We’re hiring LTSS Service Care Managers to support members with Long-Term Services & Supports (LTSS) needs. This is a hybrid role with a mix of remote documentation/admin time and in-person member visits in the field.

What you’ll do

• Manage a caseload of members needing LTSS support
• Complete member assessments (in-person and telephonic) and document detailed notes
• Develop and update individualized care/service plans
• Coordinate referrals and services, ensuring no gaps in care delivery
• Collaborate with providers, discharge planners, and other care team partners
• Support members with questions, concerns, and (as needed) complaint/appeal navigation

Schedule & work style

• Monday–Friday, 8:00 AM–5:00 PM ET
• Hybrid: typically 1–2 remote admin days/week + 3–4 field days/week
• Travel-heavy: up to ~80% travel
• Field coverage is within an up to 2.5‑hour one-way radius from the employee’s home (mileage/lodging reimbursed if needed)
• Quarterly in-person team meeting/training (Raleigh/Charlotte area)

Training requirement (important)

• 3.5 weeks of training, Monday–Friday 8:00 AM–5:00 PM ET
• Camera on during virtual training sessions
• No time off during training (please ensure you can fully commit)

Required qualifications

• Active RN license and/or LCSW / LCSW-A (must be current/valid)

• Discharge planning and transition‑of‑care experience

Not a fit if…

• Experience is psych/counseling‑only without physical health bedside/case management
• New grad / right out of nursing school
• Background is nursing home only or rehab only
