Target Start: August 24, 2026 Work Arrangement: 80% Field work/home visits with 20% remote Schedule: Monday-Friday, 8:00 AM-5:00 PM EST

Position Overview

We are seeking LTSS Service Care Managers to support healthcare members with Long-Term Services and Supports (LTSS) needs. This is a member-facing care management position responsible for managing a caseload, conducting assessments, coordinating services, developing care plans, and ensuring members receive appropriate and uninterrupted care. The role is primarily remote but requires approximately 80% travel for in-person member visits, with 1-4 visits per day depending on travel distance. Candidates must have a valid driver's license and be comfortable working in the field.

Key Responsibilities

• Manage a caseload of healthcare members with LTSS needs.
• Conduct monthly and quarterly member contacts through in-person and telephonic interactions.
• Complete member assessments and document findings in electronic medical records.
• Assess members, caregivers, and providers to understand member status, support systems, and service needs.
• Develop and update individualized care plans.
• Monitor the delivery of services and follow up with members, caregivers, and providers.
• Identify gaps in services and functional deficiencies within care plans.
• Authorize and coordinate referrals for appropriate services.
• Coordinate informal and voluntary support services as part of the member's care plan.
• Collaborate with discharge planners, physicians, and other healthcare professionals.
• Coordinate acute care and long-term care services.
• Assist members with filing and resolving complaints and appeals.

Required Qualifications

• Bachelor's degree required.
• RN and/or LCSW/LCSW-A/BSN/ADN qualification.
• 2-4+ years of physical healthcare management experience.
• Case management and care planning experience.
• Valid driver's license.
• Experience with electronic medical records.
• Strong Microsoft Office and technology skills.
• 4-6+ years of physical healthcare management experience.

Work & Travel Expectations

• Remote administrative work 1-2 days per week.
• Field/member visits 3-4 days per week.
• Approximately 80% travel.
• Up to 2.5 hours one-way from the candidate's home residence for face-to-face member visits.
• Mileage and lodging, when needed, are reimbursed.
• Candidates should reside in Pitt County, NC for this opening.
• Quarterly team meetings/training may be held at Centene offices in Raleigh or Charlotte.
