Job summary

A-Line Staffing Solutions is seeking a LTSS Service Care Manager (RN or LCSW) for a hybrid role in North Carolina. You will manage caseloads, perform in-person and telephonic assessments, and document detailed notes to develop individualized care plans.

Travel is heavy, up to 80%, with field days and remote admin time. Responsibilities include coordinating services, working with providers and discharge planners, and navigating questions or complaints.

Qualifications

• Active RN license and/or LCSW / LCSW-A (must be current/valid).
• Valid driver’s license and ability to travel for member visits.
• 2+ years of physical health care management/case management experience.
• Strong documentation skills and comfort working in electronic health record systems.
• Solid computer skills (Microsoft Office); comfortable in a virtual environment.

Responsibilities

• Manage a caseload of LTSS members and develop care/service plans.
• Complete in-person and telephonic assessments with detailed notes.
• Coordinate referrals and services, ensuring no gaps in care delivery.
