Spectraforce Technologies, Inc. seeks a Senior LTSS Service Care Manager (RN) for field-based work in the McAllen area. You will assess and coordinate medical and supporting services for complex/high acuity populations and develop personalized care plans to promote high-quality, cost-effective care.

This 6-month assignment follows a Mon-Fri 8-5 schedule with potential OT and occasional Saturdays. You will conduct member visits, collaborate with care teams, and educate families about services and

Qualifications

• RN with ASN/BSN/MSN required.
• RN license required.
• More than 3 years as nurse and 2 years in case management.
• Pediatric experience a plus.

Responsabilités

• Evaluates service needs of complex/high risk members and plans outcomes.
• Develops and updates long-term care/service plans with care teams.
• Coordinates between member/family and care providers for person-centered care.
• Monitors progress toward care plan goals and revises plans as needed.
• Educates members and families on services, options, and benefits.
• Acts as liaison between member, physician, and facilities.
