Job summary

The role requires RN or LCSW licensure and a Bachelor's degree, with 2–4+ years in health care management; hospital CM or discharge planning experience preferred. 6-month contract with 8–5 EST training and no time off during training.

Qualifications

• Bachelor's degree required with RN or LCSW licensure highly preferred.
• 2–4+ years in physical health care management; hospital CM or discharge planning experience preferred.
• Experience with long-term care services and member education is essential.

Responsibilities

• Develop, assess, and coordinate holistic care management activities for quality, cost-effective outcomes.
• Create or assist with personalized service plans for long-term care members and educate families.
• Educate members and caregivers on services and benefits available to meet needs.
