Job summary

A-Line Staffing Solutions is seeking a LTSS Service Care Manager (RN or LCSW) for a remote field-based role in North Carolina. The position involves travel to conduct face-to-face visits, manage a caseload, and coordinate services across care settings.

The ideal candidate will hold an active RN or LCSW license, have 2-4+ years of relevant experience, and be adept with EMR systems and Microsoft Office. This 6-month contract may extend or convert.

Qualifications

• Bachelor's degree required.
• Active RN or LCSW license required.
• 2-4+ years of physical healthcare case management experience.
• Experience in hospital case management, utilization management, home health or discharge planning.
• Valid driver's license and willingness to travel extensively.
• Strong Microsoft Office skills and EMR experience.
• Excellent written and verbal communication.
• Ability to work independently while managing a caseload.

Responsibilities

• Manage a caseload of LTSS-needing members.
• Conduct comprehensive assessments with members, caregivers and providers.
• Develop and monitor individualized care plans.
• Complete monthly and quarterly member contacts.
• Perform face-to-face home and community visits.
• Coordinate healthcare services with physicians, providers and discharge planners.
• Assist with discharge planning and transitions between care settings.
• Maintain documentation in EMR and case notes.
• Participate in virtual team meetings and care coordination activities.
