Job summary

A-Line Staffing Solutions is seeking LTSS Service Care Managers to support healthcare members with Long-Term Services and Supports needs. This remote-friendly role involves 80% travel for in-person visits, with 1-4 visits daily depending on distance, and 20% remote administrative work.

Responsibilities include conducting assessments, coordinating services, developing care plans, and ensuring uninterrupted care.

Qualifications

• Bachelor's degree required.
• 2-4+ years of physical healthcare management experience.
• Case management and care planning experience.
• Valid driver's license and comfortable with field work.
• Experience with electronic medical records is preferred.

Responsibilities

• Manage a caseload of healthcare members with LTSS needs.
• Conduct monthly and quarterly member contacts in person or by phone.
• Complete member assessments and document findings in EMR.
• Develop and update individualized care plans and monitor services.
• Coordinate referrals and informal support services as part of the plan.
• Collaborate with discharge planners, physicians, and other professionals.
