Job summary

Humana Inc. is seeking a Long Term Care Functional Screen Specialist to support Wisconsin Family Care programs. You will use the DHS screening tool to assess member needs and determine level of care, with field work in Milwaukee and surrounding areas.

The role requires a four-year degree or Wisconsin RN with Family Care experience, LTCFS certification, and willingness to travel up to 50%. Typical hours are Monday–Friday, 8:00 am–4:30 pm CST, with mileage reimbursement and occasional office

Qualifications

• Bachelor’s degree in a Health or Human Services field with 1 year of Family Care experience OR Wisconsin RN with 1 year of Family Care experience.
• LTCFS certification required; must complete online screener training and become certified.
• Experience with DHS screening tools and documentation practices.

Responsibilities

• Schedule member appointments for annual/change-based determinations.
• Use the DHS screening tool to assess functional abilities, health and supports.
• Enter assessment data into the online system and determine level of care.
• Submit documentation per guidelines and participate in trainings.
• Maintain screen integrity and participate in QA activities.
• Travel up to 50% within Milwaukee and surrounding areas; field work.

Education

Bachelor’s degree in Health or Human Services related field (4-year)

Wisconsin Registered Nurse license (with 1 year Family Care experience)
