Job summary

Molina Healthcare in Wisconsin is seeking a field-based care coordinator to support long-term care screening for older adults and people with disabilities. The hybrid role includes travel to member locations and monthly meetings at a Molina office.

The candidate will schedule LTCFS assessments, gather information, and work with the member and care team to ensure accurate documentation while maintaining LTCFS certification.

Qualifications

• LTCFS certification required.
• Bachelor’s degree in social work or health-related field, or equivalent.
• 1+ year in health care serving older adults or disabled.
• Valid driver’s license and reliable transportation.
• Strong communication and training abilities.
• Experience with data management and coordination.
• Ability to teach complex material to groups or individuals.

Responsibilities

• Schedule appointments for LTCFS assessments.
• Use the LTCFS to gather information about member abilities and supports.
• Complete LTCFS with member, family, and care team.
• Coordinate with ICT for enrollments and changes.
• Ensure data consistency with member records.
• Track LTCFS timeframes with spreadsheets.
• Submit documentation per guidelines.
• Attend meetings and trainings as needed.
• Participate in quality assurance activities.
• Maintain LTCFS certification through CST.
