Qualifications

• Bachelor’s degree in Health or Human Services or RN license with Family Care experience.
• LTCFS certification required.
• Previous Family Care screener experience preferred.

Responsibilities

• Schedule annual or change-based functional eligibility re-determinations.
• Use DHS screening tool to assess functional abilities and supports.
• Conduct in-home face-to-face interviews and enter data in online system.
• Submit documentation per guidelines and participate in QA activities.
• Maintain required certification through testing per Wisconsin standards.
• Travel up to 50% for field visits.
