Job summary

Humana Inc in Vernon County, WI is seeking a Long Term Care Functional Screener to support Wisconsin Family Care program members. You will conduct in-home, face-to-face assessments using the DHS screening tool to determine level of care, and schedule re-determinations.

This role requires a bachelor’s degree in Health or Human Services, LTCFS certification, and the ability to travel up to 50%; RN license in Wisconsin is an alternative.

Qualifications

• Bachelor's degree in Health or Human Services with 1 year of related experience.
• LTCFS certification required and completion of online screener training.
• Wisconsin RN license with 1 year experience in Family Care populations (alternative).

Responsibilities

• Schedule member appointments for annual or change-based functional eligibility re-determinations.
• Use the DHS screening tool to assess members' functional abilities, health, and required supports.
• Conduct in-home, face-to-face interviews and enter assessment data into the designated online system.
• Submit documentation promptly according to established guidelines.
