Job summary

Qualifications

• At least 1 year experience in health care supporting adults with physical/intellectual disabilities or the frail elderly
• LTCFS certification required
• Bachelor's degree in social work, human services or health-related field, or RN/CNA/LPN/LVN license may be required depending on state
• Valid driver's license with reliable transportation for travel requirements
• Ability to support staff development, meet performance quotas and balance team dynamics
• Experience with data management, project coordination, and training
• Ability to explain and teach complex material in group and individual settings
• Excellent verbal and written communication skills
• Microsoft Office suite proficiency

Responsibilities

• Schedules appointments with members to complete LTCFS assessments
• Uses Wisconsin LTCFS to gather information on member status and supports
• Completes LTCFS collaboratively with member, family, and ICT
• Coordinates with ICT to ensure enrollment and condition screens are completed
• Ensures data consistency between assessment data and member records
• Uses spreadsheets to track LTCFS timelines
• Submits timely, accurate documentation per guidelines
• Attends meetings and training to stay updated on processes
• Participates in quality assurance activities and audits
• Maintains LTCFS certification by passing CST
• Applies professional judgment while documenting assessments
• Local travel may be required based on state/contractual requirements
