Job summary

Molina Healthcare is seeking a care coordinator to support long-term care screening processes for older adults and individuals with disabilities in Wisconsin. This hybrid role requires travel to member locations and monthly meetings at a Molina office.

The successful candidate will schedule LTCFS assessments, gather functional data, coordinate with the ICT, ensure data accuracy, and maintain LTCFS certification through CST.

Qualifications

• At least 1 year of health care experience serving older adults or people with disabilities.
• LTCFS certification is required.
• Bachelor's degree in social work/health field or active RN/CNA/LPN license may be required by state.
• Valid driver's license and reliable transportation.
• Experience with data management, project coordination, and training.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office applications.

Responsibilities

• Schedules appointments for LTCFS assessments.
• Uses the Wisconsin LTCFS to gather information about member abilities and status.
• Completes the LTCFS in collaboration with the member, family, and ICT.
• Coordinates with member ICT for enrollment, annual reviews, and changes.
• Ensures data consistency between assessments and member records.
• Uses spreadsheets to track LTCFS timeframes.
• Submits timely, accurate documentation per guidelines.
• Attends meetings and training to maintain knowledge of processes and tools.
• Participates in quality assurance activities to ensure competence.
• Maintains LTCFS certification by CST testing.
• Occasional local travel may be required.
