Job summary

Molina Healthcare seeks a qualified care coordinator to support long-term care functional screening (LTCFS) for older adults and adults with disabilities across Wisconsin. This hybrid role includes travel to member locations and monthly meetings at a Molina office.

Responsibilities include scheduling LTCFS assessments, collecting health information, coordinating with the ICT, and ensuring accurate documentation while maintaining LTCFS certification.

Qualifications

• At least 1 year experience in health care supporting adults with disabilities or frail elderly.
• LTCFS certification is required.
• Bachelor's degree in social work, human services or health-related field, or RN/CNA/LPN/LVN license may be required depending on state.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.

Responsibilities

• Schedule appointments for home-based LTCFS assessments.
• Gather information on member abilities, health status, and supports.
• Complete LTCFS in collaboration with member, family, and ICT.
• Coordinate with ICT to ensure enrollments and condition screens are completed.
• Maintain consistency between assessment data and member records.
• Use spreadsheets to track LTCFS timeframes.
• Submit timely and accurate documentation per guidelines.
• Attend meetings and trainings to stay updated on processes.
• Participate in quality assurance and audits.
• Maintain LTCFS certification through CST.
