Job summary

Wellbox seeks a Clinical Care Coordinator (LPN/LVN) to perform high-volume, telephonic care coordination. You will work remotely across the continental US, handling structured patient calls, documenting in real time, and meeting strict CE targets.

The role emphasizes speed and accuracy, with a ramp-up period and performance-driven metrics. A nursing license and remote-work readiness are essential.

Qualifications

• Active LPN/LVN license (compact if available)
• Licensed in your state of residence; must reside in continental US
• Minimum 2 years of clinical experience
• Comfortable with EHR documentation and Microsoft Office
• Experience in telephonic care coordination, health coaching, or motivational interviewing is a plus

Responsibilities

• Make back-to-back telephonic patient calls during 8-hour shifts
• Document each encounter in real-time within the EHR
• Meet productivity targets (1.5 Care Events per hour after ramp-up)
• Follow a defined care coordination framework with structured conversations
• Operate in a focused, phone-based role with minimal administrative variety
